Our Services
Comprehensive Nursing
At Little Village Nursing and Rehabilitation Center, the core of everything we do is to help our residents regain their health, independence, and quality of life to their highest abilities. Our team of dedicated nurses strives to serve our residents with personalized, goal-oriented care 24 hours a day, 7 days a week.
Long-term Care
We fully support residents and their families as they transition from the comforts of their homes to Little Village Nursing and Rehabilitation Center. Residents will be involved in therapy as prescribed by their physician as well as immersed in daily activities to stimulate their social and emotional well-being. Residents will be supported by our staff to meet all of their personal care needs. These can include bathing, dressing, toileting, transferring, incontinence care, eating assistance, and medication reminders.
Social Services
Our social service team provides essential information, manages requests and concerns, and helps in care planning and discharge planning for each resident. We can arrange professional services for psychosocial and psychiatric evaluation, individual and/or group therapy, substance abuse program, power of attorney, discharge planning, assistance with obtaining personal identification documents, and conflict resolution.
Diabetes Management
Our experienced nurses provide help with all aspects of managing diabetes including blood glucose monitoring, insulin administration and tracking, exercise planning, menu planning, education, and more.
Dietary Program
Our dietary program is designed to meet the dietary and physician-ordered needs of each resident in our care. To ensure that each resident is receiving the proper nutritional needs we offer meals to meet specific dietary and therapeutic needs, according to physician orders, oral supplemental, as prescribed by each physician, and regular nutritional assessments from our registered dietitian.
Activities
A positive and uplifting atmosphere can set the tone for a resident’s stay. Here at Kensington Place, each resident is given a calendar to keep them informed of upcoming events such as birthday parties, movies, musical programs, outings, and other special events. We want our residents to stay as social and active as they can be.

Our rehabilitation programs are designed to assist each resident in increasing, restoring, or maintaining range of motion, physical strength, flexibility, coordination, balance, and endurance following an injury, illness, or surgery. This program will also help the residents achieve an independent, productive, and satisfying life as they work to improve their ability to carry out activities of daily living. Physical, occupational, and speech therapy is provided by our in-house select rehab team.

Personalize Care Plans
Wound Care
Little Village Nursing and Rehabilitation Center has a physician-led wound care team that provides our residents with daily wound care. Our Wound Care Physician makes weekly rounds and meets with the wound care team to assure that our residents’ wounds are healing properly. Wounds require special medical attention and our certified wound care nurse works with our wound care team to make sure they provide the proper assessments and treatment plans to promote healing.
We care for wounds gently by developing a custom treatment plan fit for each individual in their unique stage. Offering bedside debridement and wound care treatments, our specialized team monitors each wound site for signs of infection for proper healing and recovery. Our wound care team has worked with post-surgical wounds, venous and arterial stasis wounds, and more.

Mental Healthcare
Behavioral Care
Little Village offers unique programming for individuals with behavioral needs. Whether it be short-term rehabilitation, medication management, anger management, life skills, or community reintegration, we offer a full range of services to fit each individual’s needs. Our psychiatric rehabilitation is performed by professional mental health staff.
Community Integration Program
Little Village helps transition residents to return to a more independent living environment within the community. Our professionally trained counselors work with residents, utilizing a structured behavioral management program. Each resident has an individualized care plan enabling them to achieve their highest level of opportunity.
The ultimate goal of the transition is to provide the skills to our residents to function on a daily basis with treatment, vocational skills training, and independent living.
Therapeutic Groups
We are proud to offer Therapeutic Groups for our residents here at Little Village Nursing and Rehabilitation Center. This program offers our residents, various therapy groups, to deal with the many challenges that can occur in one’s life.
